Male Survivors of Sexual Abuse: Confidential Questionnaire

The Ontario Ministry of the Attorney General is committed to building new province-wide services for male survivors of sexual abuse.

In order to achieve this goal, we have worked with clinicians and other experts to develop a confidential questionnaire that will help us gather important advice from male survivors of sexual abuse.  If you are a male survivor of sexual abuse, which includes being abused as a child, and are a past or present resident of Ontario, we want to hear from you.

Please download the confidential questionnaire below and share your advice about the types of services that you believe would help you in the future.  You will not be required to provide your name or address to fill out the questionnaire.  We ask that you submit your completed responses by January 17, 2011. However we will always welcome your feedback at any time.  Please submit your completed responses to:

We recommend that you send your completed questionnaire by mail to ensure confidentiality.  You may also e-mail your completed questionnaire, but please note that using the Internet is not as secure as mailing in your response.  If you choose to e-mail your completed questionnaire, you may send it to: ovss.responses@ontario.ca

If you require a hard copy of the questionnaire, or if you require assistance to provide your responses, please call James Truman at (416) 212-1107.  Collect calls will be accepted.

If you are an organization providing victim services, mental health services or social services to male survivors, please inform them about the questionnaire.

Ontario is committed to moving forward with its plan to provide services for male survivors of sexual abuse.  As the first initiative of its kind in Canada, we want to work together to develop the best combination of services.
