Please take or send this completed form to the court office where the court order was made. The address and telephone numbers of the various court offices may be found at: www.attorneygeneral.jus.gov.on.ca or in the blue pages of your telephone directory.

Please also note that there is a court fee to make a copy of your order. Court office staff will tell you the amount you need to pay and how to make payment once your completed request is received. If you cannot afford to pay the court fee, you may request a fee waiver. Fee waiver request forms and a guide to the fee waiver process and financial eligibility criteria are available at any court office, or on the Ministry of the Attorney General website at www.attorneygeneral.jus.gov.on.ca.
