Form for Arbitrators of Family Disputes
The Family Arbitration Form is available in PDF Format at the Ministry of Government and Consumer Services Forms Repository Site, or in HTML format at the Accessible Forms site. The PDF form can be completed online and submitted electronically or printed out and mailed or faxed. The HMTL form can also be completed online but submitted only by printing it, after which it can be mailed or faxed. Both forms can be printed out in blank and completed on paper, then mailed or faxed. You may want to print out the instructions for using the form, before accessing the form.
