Form for Arbitrators of Family Disputes

Arbitrators have the option of completing the Family Arbitration Report in one of three ways:

  1. Electronic version in Portable Document Format (PDF) - Arbitrators may fill out the report in Adobe Reader and submit it by e-mail.  To do so, the arbitrator must complete the form in its entirety, and then hit the “Submit by Email” button within the form.  If the arbitrator wishes to retain a copy of the report for his or her records, they should press the “Print Form” button prior to submitting it.  Note that the report cannot be saved electronically. 

  2. Electronic version in Hypertext Markup Language (HTML) - Arbitrators may fill out the report in their browser, and then print the report and mail or fax it to the address shown in paragraph 3) below. Note that the report cannot be saved electronically.

  3. Paper version - Arbitrators may print out the report to complete it by hand (note that to do this the print button on the toolbar must be used, not the print button within the form). The completed form may be submitted by mail or fax to the following address:

Once a report has been submitted, it will be assigned a Ministry File Number and then a copy will be returned to the arbitrator for his or her records.
