Victim Services Awards of Distinction Nomination Form
NOMINATIONS CLOSED: The nomination period for the Attorney General's Victim Services Awards of Distinction ended Friday, October 9, 2009. The ministry would like to thank all those who submitted nominations. Award recipients will be notified when the nominations have been reviewed and final decisions have been made.
The Attorney General's Victim Services Awards of Distinction Nomination Form is available in Microsoft Word and Adobe Acrobat format.
While the form can be filled out electronically, it must be printed out, signed, and submitted via letter post. Due to the need for original signatures, the form cannot be submitted electronically (e.g. via fax or e-mail).
If you wish to fill out the form over several sessions, you must save the file on your hard drive and re-open it to continue editing the submission. Otherwise, your changes may be lost.
