The Attorney General's Victim Services Awards of Distinction Nomination Form is available in Microsoft Word and Adobe Acrobat format.

While the form can be filled out electronically, it must be printed out, signed, and submitted via letter post. Due to the need for original signatures, the form cannot be submitted electronically (e.g. via fax or e-mail).

If you wish to fill out the form over several sessions, you must save the file on your hard drive and re-open it to continue editing the submission. Otherwise, your changes may be lost.
